   Intensive / Critical Care Medicine.

Анотація: This book is designed to provide a comprehensive and state-of-the-art resource for clinicians who care for patients with sepsis and research scientist alike, . Patients with severe sepsis requiring ICU admission have very high rates of ICU and overall hospital mortality, with estimates ranging from 18 to 50%. Risk factors for death from sepsis include underlying illness, increased age, and multi-system organ failure. This is compounded by the significant variation in the management of early severe sepsis. Care of these patients and clinical conditions can be quite complex, and materials are collected from the most current, evidence-based resources. Book sections have been structured to review the overall definitions and epidemiology of sepsis as well as current insights into the pathophysiology of sepsis. This review summarizes the evidence for the international consensus guidelines for the identification and management of sepsis. The latter part of this book reviews emerging concepts and approaches in the diagnosis and management of sepsis that may significantly reduce mortality in the future. Sepsis: Pathophysiology, Definitions and the Challenge of Bedside Management represents a collaboration between authors drawn from a variety of disciplines and contributions from basic scientists and highly recognized clinical opinion leaders with expertise in clinical trials.

Анотація: This book offers a comprehensive, up-to-date overview of optimal postoperative care in patients who have undergone thoracic surgery and discusses challenging issues that are of interest not only in the context of thoracic surgery but also more generally within the fields of anesthesia, intensive care, and pain medicine. The coverage ranges, for example, from use of non-invasive ventilation, extracorporeal membrane oxygenation, and new monitoring devices to fluid management, pain control, and treatment of arrhythmias. A key feature of the book is the exceptionally attractive list of authors, who represent the most authoritative experts on the topics that they address. The approach is appropriately multidisciplinary, acknowledging that in many centers thoracic anesthetists are responsible for care during the postoperative period. This book is exceptional in being devoted solely to postoperative care in thoracic surgery, even though the difficulties involved in thoracic operations sometimes exceed those of cardiac surgery, especially in the postoperative period.

Анотація: This manual is designed to serve as the standard for the management of patients with ruptured abdominal aortic aneurysm (AAA) by means of modern techniques. All aspects are covered, from pathogenesis, clinical presentation, and diagnosis through to initial management, operative strategies, postoperative complications, outcomes, and quality of life issues. The book is based on the experience gained at a leading U.S. institution (Harborview Medical Center, University of Washington) that treats up to 40 patients with ruptured AAA annually and is highly respected for its expertise in this medical emergency. The team at Harborview Medical Center has recently shown that the implementation of a structured protocol for the management of ruptured AAA, with emphasis on an “endovascular-first” strategy, dramatically reduces mortality and complications. This has led to a sea change in the way in which patients are managed. There is an increasing hunger for information on these codified protocols, and Ruptured Abdominal Aortic Aneurysm: The Definitive Manual will be invaluable for both surgeons and critical care physicians.

Анотація: This book discusses, explains and provides detailed, up-to-date information on physics applied to clinical practice in anesthesiology, with the aid of simple examples from daily life. Almost everything that happens around us, including in the operating room and intensive care units, can be explained by physical laws. An awareness and understanding of relatively simple laws such as Bernoulli’s theorem, Hagen-Poiseuille equation and Pascal’s principle, to name just a few, offer anesthesiologists and intensivists fascinating insights into why they do what they do. Each of the 16 chapters starts with an everyday phenomenon, explains it with a physical law, and then shows why that law is important in anesthesia practice. Numerous illustrations are included for extra clarity. It is intended for anesthesiologists, intensivists, anesthesia teachers, anesthesia trainees, and medical students.

    Evidence-Based Critical Care [Electronic resource] : a Case Study Approach / / ed. Hyzy, Robert C. - 1st ed. 2017. - [S. l. : s. n.]. - XXVI, 805 p. 233 illus., 133 illus. in color. - ISBN 9783319433417
    Зміст:

Анотація: This textbook approaches the study of critical care with teaching in mind. In medicine “teachable moments” usually occur in clinical context, where the engagement in a real case exemplifies principles of diagnosis or therapy. In order to replicate the teaching method, selected cases are presented within each chapter offering the reader an opportunity to process and reflect on the components of the topic within a practical scenario. While medical practice attempts to be evidence-based, common approaches to diagnosis and management incorporate not only evidence but heuristics and biases which await either validation or repudiation. Hence, each chapter is divided into two segments: the “Principles of Management” section – the common approach to the care of patients having a given condition is presented – and the “Evidence Contour” section, in which each author discusses the aspects of diagnosis and management that are the subject of ongoing debate. Evidence-Based Critical Care: A Case Study Approach bridges the gulf between the traditional encyclopedic textbook and the single-hit online repository. This approach will help critical care practitioners, fellows, residents, allied health professionals and students expand their critical care knowledge in an efficient and effective manner, while being especially important for those preparing for board examinations.

Анотація: This text covers the major controversies and "myths" in each of the major anesthesia subspecialties. You're Wrong, I'm Right is designed to be an easy and engaging evidence based read that offers the fast-paced give-and-take of a debate between two experts at the top of their game--capturing their full argument, including expressions of humor and displays of temper. Each point of contention begins with a real case, carefully selected to encapsulate the argument. One author then argues the "pro" side and another the "con." Sometimes a single author may argue both sides. In doing so, the authors highlight the newest evidence and remind us of classic principles that have stood the test of time. At the end of the debate, readers can determine which argument they will use in their clinical practice, and may also consult a final "Consensus” section that identifies the editors' and contributors' "picks" of the one best practice in a range of different situations. .

Анотація: This book is designed to present a comprehensive, state-of the-art approach to assessing and managing bariatric surgery and psychosocial care. Unlike any other text, this book focuses on developing a biopsychosocial understanding of patients’ obesity journey and psychosocial factors contributing to their obesity and its management from an integrated perspective. Psychiatric Care in Severe Obesity takes a 360 approach by covering the disease’s prevalence and relationship to psychiatric illness and social factors, including genetics, neurohormonal pathways and development factors for obesity. This book presents evidence and strategies for assessing psychiatric issues in severe obesity and uses common psychiatric presentations to feature the impact on bariatric surgery and key assessment features for weight loss. Concluding chapters focus on evidence-based psychosocial treatments for supporting patients with weight loss and bariatric surgery and includes educational tools and checklists for assessment, treatment, and care. Experts on non-pharmacological interventions such as mindfulness, cognitive-behavioral therapy and nutrition education describe treatment approaches in each modality, concluding with pharmacological approaches for psychiatric conditions and eating pathology. Additional tools in the appendices support clinicians, making this the ultimate guide for managing psychiatric illness in patients suffering from severe obesity. As obesity continues to grow in prevalence as a medically recognized epidemic, Psychiatric Care in Severe Obesity serves a vital resource to medical students, psychiatrists, psychologists, bariatric surgeons, primary care physicians, dietitians, mental health nurses, social workers, and all medical professionals working with severely obese patients.

Анотація: This book considers paradigmatic clinical cases in order to cast light on key issues relating to elective or emergency stent implantation and the use of oral anticoagulation (OAC) in patients with atrial fibrillation. The topics addressed include the optimal periprocedural antithrombotic treatment (uninterrupted vs interrupted OAC, intraprocedural use of heparin and glycoprotein IIb/IIIa inhibitors, etc.), the most appropriate type of stent (bare metal vs drug eluting vs “bioactive”), the optimal regimen (e.g., triple therapy of OAC, aspirin, and clopidogrel vs the combination of OAC and a single antiplatelet agent), and the most suitable duration of the antithrombotic treatment prescribed at discharge (1 vs 6–12 months). The case-based management recommendations will be of wide practical value in the current health care context, where percutaneous coronary intervention is available even to patients with relevant co-morbidities, such as those warranting long-term OAC, and the indications for OAC are much broader than in the past. The book will appeal especially to clinical and interventional cardiologists, internal medicine specialists, hematologists, and family physicians and will also be of interest to cardiology and internal medicine residents and fellows.

Анотація: This unique and richly illustrated volume presents the state of the art in the comprehensive management of major neurosurgical diseases in the elderly (aged 65 and over). It explores all of the common neurosurgical pathologies affecting elderly patients, and emphasizes the paramount importance of tailored management strategies for quality of life. It highlights updated techniques for anaesthesia and critical care, as well as minimally invasive neurosurgical methods intended for this specific group of patients. Radiosurgery treatment is also discussed, in particular for brain tumours. In western societies, the proportion of elderly citizens has nearly reached 20%, and shows no signs of slowing down. The management of neurosurgical conditions in this particular population requires specific multidisciplinary strategies. To address this situation, a team of internationally respected contributors accurately describe degenerative and traumatic spinal diseases, which account for the majority of admissions among the elderly, as well as brain tumours and intracranial haemorrhages, aspects that are raising new ethical issues. The book mainly addresses the needs of neurosurgeons and geriatric neurologists, but also neuro-oncologists and neuro-anaesthesists working with elderly patients, as well as students in these disciplines.

Анотація: Those familiar with the 1st edition of this book from 2008 will recognize the original 62 cases that have been modernized with updated references and enjoy the addition of 41 brand new cases. Clinical Anesthesia: Near Misses and Lessons Learned, 2nd edition is a collection of actual cases, complied from the author’s forty years of practice in major metropolitan hospitals in the United States, Norway and South Africa. It offers the reader succinct case presentations describing a problem on one page and a solution on the next, with a discussion, other potential solutions with satisfactory outcomes, and tips to help avoid problems altogether when possible. Clinical Anesthesia: Near Misses and Lessons Learned, 2nd edition serves as an easy and practical way for the reader to gain familiarity with potentially unexpected problems in clinical anesthesia In addition, this can be an excellent study aid for the American Board of Anesthesiology oral exam. Since near misses are relatively rare, there really is no way to be prepared to successfully manage such crises other than reading about them, so residents, fellows, and practicing, certified registered nurse anesthetists will benefit from the learning of these actual near misses. Additionally, faculty will find the plethora of discussion topics for large or small group settings an ideal way to communicate anesthesia and clinical care problems to the audience.
