Анотація: This informative text details the many changes in everyday life as the result of injury, illness, or aging affecting the brain. Experts across brain-related fields trace mechanisms of conditions such as Parkinson’s disease, TBI, and dementia as they impact regions of the brain, and resulting cognitive, emotional, sensory, and motor impairments as they contribute to deficits in personal and social functioning. In addition to symptoms and behaviors associated with insults to the brain (and the extent to which the brain can adapt or self-repair), chapters provide cogent examples of how societal and cultural expectations can shape the context and experience of disability. The book’s focus on everyday activities brings new clarity to diverse links between symptoms and diagnosis, brain and behavior. Included in the coverage: ·The aging brain and changes in daily function. ·Stroke: impact on life and daily function. ·Traumatic brain injury (TBI) and the impact on daily life. ·Everyday life with cancer. ·Real-world impact of HIV-associated neurocognitive impairment. ·Disability and public policy in America. ·Living after brain changes, from the patient’s perspective. Rich in empirical data and human insight, Changes in the Brain gives neuropsychologists, clinical psychologists, clinical social workers, and rehabilitation nurses a robust new understanding of the daily lives of patients, both in theory and in the real world.

Анотація: This book tells the intriguing and often colorful stories of the medical words we use. The origins of clinical and scientific terms can be found in Greek and Latin myths, in places such as jungles of Uganda and the islands of the Aegean Sea, in the names of medicine’s giants such as Hippocrates and Osler, and in some truly unlikely sources. In this book you will learn the answers to questions such as: • What disease was named for an American space flight? • Do you know the echoic word for elephantine rumbling of the bowels? • What drug name was determined by drawing chemists’ notes out of a hat? • What are surfer’s eye, clam digger’s itch, and hide porter’s disease? This book can give you new insights into the terms we use every day in the clinic, hospital, and laboratory. Knowing a word’s history assists in understanding not only what it means, but also some of the connotative subtleties of terms used in diagnosis and treatment. The Amazing Language of Medicine is intended for the enrichment of physicians, other health professionals, students, and anyone involved in clinical care and medical science.

Анотація: This book focuses on clinical presentations that may be caused by inherited metabolic diseases. Its symptom- and system-based approach will help clinicians with and without detailed knowledge of human biochemistry in all specialties to reach a correct diagnosis and institute the optimal treatment program. The book summarizes the central elements of inherited metabolic diseases and describes clearly how to carry out an efficient yet complete diagnostic work-up, thereby guiding the clinician from the presenting symptoms and signs through to effective initial management. After an introduction to the different disorders, the book explains when to consider an inborn metabolic error and which initial tests to order. Core aspects such as structured communication, guidelines, transition, pregnancy, maternal care and how to respond to various medical emergencies are covered. Therapeutic concepts such as dietary treatment are delineated and practical advice provided on the quite different treatment approaches required for individual diseases. An extensive section structured according to organ systems outlines the correct approach in the context of specific symptoms and signs. The value of each of the potential investigations is explained, with precise advice on the interpretation of results. The inclusion of algorithms, tables, lists, and charts facilitates rapid decision making and information retrieval, and the appendices include a helpful guide to differential diagnosis based on clinical and biochemical phenotypes. This new updated edition of Inherited Metabolic Diseases will be an invaluable aid for the busy clinician and an excellent quick reference for metabolic and genetic specialists.

Анотація: This new edition now titled “Human Chromosome Variation: Heteromorphism, Polymorphism and Pathogenesis” provides the reader with an up-to-date overview of microarrays, fragile sites, copy number variations and whole genome sequencing. Greatly expanding the discussion of microarray analysis in the previous edition of the book, are new chapters on microarray and genomic analysis, plus comprehensive tables on the subtle microdeletions and microduplications that are found on each chromosome, including 235 recurring copy number variants that are associated with well-established or emerging chromosomal syndromes. The current edition features concise information on cytogenetic methods and applications, extending these discussions to DNA analysis and genome sequencing. Sections on euchromatin, heterochromatin, FISH pattern, fragile site, copy number, and DNA sequence variation are integrated with actual clinical examples from cytogenetic laboratories and from clinical practice. The principles that allow for the distinction between benign chromosome / DNA variation and pathogenic heteromorphisms / polymorphisms are discussed and include references to the latest organizational guidelines and genomic or population databases. The two previous incarnations of this book: the ‘Atlas of Human Chromosome Heteromorphism’, and ‘Human Chromosome Variation: Heteromorphism and Polymorphism’ have been standard reference works in most cytogenetic laboratories, used by laboratory directors and clinicians all around the world. While widely used sections from the previous edition on cytogenetic technologies and heteromorphisms are retained intact the present volume adds extensive material on copy number variations (polymorphisms detected by microarray analysis), fragile sites in disease and cancer, and practical views on interpreting emerging technologies, including whole exome sequencing. This book should be of interest to clinicians, technicians and students who are or will be exposed to DNA and/or chromosome analysis and the data derived from these continuously developing techniques. This fully updated book volume will bring the reader up to speed on the latest technologies, their applications, benefits and drawbacks and as such, is a must read for anyone with an interest in DNA and chromosome analysis and the distinction between benign variation and pathogenic mistakes.

   Primary Care Medicine.

Анотація: This book provides a comprehensive view of rational suicide in the elderly, a group that has nearly twice the rate of suicide when chronically ill than any other demographic. Views of rational suicide are evolving, particularly because of a growing older population with a longer life expectancy. However, there is little guidance for geriatric mental health professionals about this. The book's frame of reference does not endorse a single point-of-view about the legitimacy of rational suicide, but serves as a resource for both those clinicians who agree that older people may rationally commit suicide and those who believe that this wish may require further assessment and treatment. The first chapters of the book address the question of whether suicide in the elderly can be rational from bio-ethical and clinical perspectives and whether the nearness of death in late-life adults means that clinicians should approach suicide differently in that group than in younger adults. Clinical criteria for rational suicide in the elderly are proposed in this book for the first time. Another chapter addresses legal issues for clinicians treating older adults who wish a rational end of life. The book examines rational suicide in the elderly through history and across cultures, while also addressing the special case of Baby Boomers. Unlike any other book, this text examines the issue from existential, spiritual, psychological, and psychodynamic perspectives. Another chapter takes a novel look at the possibility of terminal mental illness and how this relates to suicide. This book is the first to consider psychotherapeutic, spiritual, and pharmacological interventions for an older adult who is contemplating suicide without a diagnosable mental illness. The book concludes with a commentary on modern society, aging, and rational suicide that ties all of these elements together, making this the ultimate guide for addressing rational desires for suicide among the elderly. Rational Suicide in the Elderly is an excellent resource for all medical professionals with potentially suicidal patients, including geriatricians, geriatric and general psychiatrists, geriatric nurses, social workers, and public health officials.

Анотація: This book provides medical students and physicians with a practical, step-by-step guide on how to write and publish a medical case report. The case report is the traditional way for physicians to describe their unique or unusual cases to a broad audience and it plays an important role in the discovery of new diseases or syndromes, unusual manifestations of disease, important adverse drug reactions, and the generation of hypotheses for further study. This book guides readers through the process from choosing a case to report on to finding a publisher and then comment on future directions and potential new uses of case reports, including expanded computer case databases to optimize care for individual patients and new applications in medical education. Interspersed throughout the text are example case reports, many written by the authors, with commentary on their experiences working with those reports to provide context and aid readers in creating clear, concise, and useful case reports. .

Анотація: This book focuses on the fundamentals of the use of biologics in asthma, describing the rationale, principles, mechanisms of action, and indications. It offers an excellent balance between basic science and the analysis of clinical trials, updating readers with new developments that are changing the global scenario for targeted biological anti-asthma therapies, especially with regard to more severe disease. A range of therapies are considered, from the humanized monoclonal anti-IgE antibody omalizumab, widely approved as add-on treatment for inadequately controlled disease, through to emerging biologics for which evidence supportive of efficacy is accumulating, including anti-IL-5, anti-IL-4, and anti-IL-13 therapies. One aspect to emerge is the variability in individual response, which suggests a need for characterization of different asthma subtypes to permit the effective implementation of phenotype-targeted treatments. This book will be of interest for pulmonologists, clinical immunologists, and physicians seeking sound information on these therapies, but also for scientists and pharmacologists wishing to enhance their knowledge of the therapeutic implications of the cellular and molecular mechanisms that underlie severe, uncontrolled asthma.

Анотація: The second edition of this definitive guide for clinical care of adolescents builds upon the practical knowledge and guidance of the first edition, and expands into new subjects of adolescent care. The handbook is divided into three sections: general adolescent medicine, sexuality, and mental health, and contains relevant, practical knowledge, covering those areas most often seen in the practice of adolescent medicine. The MassGeneral Hospital for Children Adolescent Medicine Handbook, 2nd edition details best practices in regards to diagnostic evaluations and clinical care, but also instructs practitioners on the best methods to connect, communicate, and continue that care with adolescents, in order to provide optimal treatment, and instill healthy lifetime behaviors. Each chapter is written by clinicians who have been trained at, or are members of the staff of Massachusetts General Hospital, and this edition has nearly doubled the amount of skilled physician authors. While this title has been revised and updated, entirely new chapters devoted to hypertension, immunizations, breast disorders, HIV, and resilience have also been added, reflecting new and changing contributions to the field of adolescent medicine. This second edition brings together the practical, hands-on knowledge of the first edition, along with new information and additional subject areas to create a balanced, multi-specialty method to treating  and engaging adolescent patients.

Анотація: This volume describes the basics for short- and long-term treatment of Psychogenic Non-Epileptic Seizures (PNES) in children. The text acknowledges that the disorder, though rare and highly morbid, is treatable when it is not misdiagnosed. Given the limited diagnostic and clinical training offered to clinicians, this book aims to equip professionals with the tools needed to improve the poor quality of life of youth with PNES. The text begins by introducing the main features of the disorder and the problems involved in diagnosing PNES in children. It then describes techniques to overcome these problems in order to make a reliable and valid diagnosis of PNES, as well as provide feedback on the diagnosis and treatment plan. The last section describes the indications for cognitive behavior therapy for youth with PNES and suggested treatment paradigms. Анотація: This landmark text is dedicated to the conversation had between a primary care physician and a dermatologist that is a true reflection of the way medicine works when these physicians collaborate on the same case. The book uses actual case studies from the authors' offices and provides an accurate and real portrayal of the types of skin conditions primary care physicians encounter. Answering questions such as when to treat, how to treat, when to refer, when to biopsy, and when to reassure, this book informs and educates primary care physicians with a dermatologist’s perspective. Top 50 Dermatology Case Studies for Primary Care gives the reader an entirely new vantage point from which to view dermatologic cases, and together with one of New York City's top dermatologists, the authors look at the same case, the same patient, and compare what each did or would have done. Анотація: This book is designed to present a comprehensive, state-of the-art approach to assessing and managing bariatric surgery and psychosocial care. Unlike any other text, this book focuses on developing a biopsychosocial understanding of patients’ obesity journey and psychosocial factors contributing to their obesity and its management from an integrated perspective. Psychiatric Care in Severe Obesity takes a 360 approach by covering the disease’s prevalence and relationship to psychiatric illness and social factors, including genetics, neurohormonal pathways and development factors for obesity. This book presents evidence and strategies for assessing psychiatric issues in severe obesity and uses common psychiatric presentations to feature the impact on bariatric surgery and key assessment features for weight loss. Concluding chapters focus on evidence-based psychosocial treatments for supporting patients with weight loss and bariatric surgery and includes educational tools and checklists for assessment, treatment, and care. Experts on non-pharmacological interventions such as mindfulness, cognitive-behavioral therapy and nutrition education describe treatment approaches in each modality, concluding with pharmacological approaches for psychiatric conditions and eating pathology. Additional tools in the appendices support clinicians, making this the ultimate guide for managing psychiatric illness in patients suffering from severe obesity. As obesity continues to grow in prevalence as a medically recognized epidemic, Psychiatric Care in Severe Obesity serves a vital resource to medical students, psychiatrists, psychologists, bariatric surgeons, primary care physicians, dietitians, mental health nurses, social workers, and all medical professionals working with severely obese patients.

Анотація: This book has two goals: to educate healthcare professionals about the effect of identity-based adversity on the health of their LGBT patients, and to outline how providers can use the clinical encounter to promote LGBT patients’ resilience in the face of adversity and thereby facilitate recovery. Toward this end, it addresses trauma in LGBT populations; factors that contribute to resilience both across the lifespan and in specific groups; and strategies for promoting resilience in clinical practice. Each chapter includes a case scenario with discussion questions and practice points that highlight critical clinical best practices. The editors and contributors are respected experts on the health of LGBT people, and the book will be a “first of its kind” resource for all clinicians who wish to become better educated about, and provide high quality healthcare to, their LGBT patients.

Анотація: With the proliferation of research studies posted online, media outlets scrambling to pick up stories, and individuals posting unverified information via social media, the landscape for parents trying to understand the latest science as it pertains to their children has never been more challenging to navigate. This book is intended to assist pediatricians when discussing research findings with parents. It provides an overview of research practices and terminology, clarifies misconceptions about studies and findings, and explains the limitations of research when applied to medical decision making. Through this framework, physicians can explain their reasoning behind specific clinical recommendations. In addition to examining the broad concepts comprising research literacy, this book reviews the current findings in topics that pediatricians report discussing most often with parents, such as vaccines, diet, medications, and sleep. Pediatrician’s Guide to Discussing Research with Patients is a unique resource for pediatricians in encouraging the development of research literacy in their patients.
