Geared to oncologists and researchers, but nonetheless a good foundation resource for patients and families who wish to learn a great deal about lung cancer and its management.Lung cancer remains the leading cause of cancer-related death in men, and in women it has surpassed even breast cancer. The primary cause of lung cancer in up to 90 percent of patients is smoking, and it is estimated that bronchiogenic carcinoma will develop in 10 to 15 percent of all smokers. Environmental factors — for example, passive smoke, occupational exposure to asbestos or chromium, and air pollution — as well as genetic predisposition in a multistep carcinogenic process may both play a part. Nevertheless, cessation of cigarette smoking is the most effective, most easily achievable, and least expensive intervention in reducing the risk of lung cancer. Despite marked progress in the treatment of lung cancer, most patients still die of their disease within several months to a few years. Some patients and physicians have a nihilistic perception of the treatment of lung cancer, and adequate treatment is often delayed or not even considered. However, even locally advanced disease can be cured in many patients with the appropriate multimodal therapy. Patients with a suspected or confirmed diagnosis of lung cancer are best evaluated by a team of lung-cancer specialists consisting of thoracic surgeons, medical oncologists, radiation therapists, pneumologists, radiologists, and pathologists. Lung Cancer: Principles and Practice reflects this multidisciplinary approach to a common disease. Lung cancer should be suspected in any patient who presents with persistent cough or hemoptysis or with deep venous thrombosis, without other clinical explanations or constitutional symptoms. A simple chest radiograph may reveal a pulmonary mass, and subsequent computed tomography of the chest and upper abdomen will allow rapid assessment of any extension of the tumor into the mediastinum and of any metastases to the liver and adrenals. Tissue diagnosis should then be performed by the most appropriate means (e.g., transthoracic fine-needle biopsy, transbronchial biopsy, or mediastinoscopy). The advantages and appropriateness of each technique are discussed in detail in this book. Once the histology and precise stage of the tumor are determined, the optimal treatment and sequence of interventions can be discussed. Approximately half the book is devoted to clinical management by surgery, radiation therapy, and chemotherapy. Small-cell lung cancer and non–small-cell lung cancer are discussed separately, and treatment recommendations are given according to stage and other prognostic factors. Approximately half the patients present with metastatic disease at diagnosis, and chemotherapy is usually indicated. The emerging role of chemotherapy not only in the treatment of small-cell lung cancer but also in the treatment of non–small-cell lung cancer is reflected by the inclusion in separate chapters of a discussion of the role of second-line chemotherapy and the treatment of elderly persons. An entire section of this textbook is devoted to additional, palliative treatments such as radiotherapy and brachytherapy, the use of expandable stents and laser surgery for airway obstruction, and the management of pleural effusion. Most patients in whom treatment can be administered with curative intent present with locally advanced disease. In these patients, (surgery alone) is rarely sufficient, and a multimodal approach to treatment should be considered. Although adjuvant chemotherapy has failed to offer a survival advantage, modern combination chemotherapy has shown promise as neoadjuvant therapy before surgery or radiation. The indications and limitations of adjuvant radiotherapy after complete resection of non–small-cell lung cancer are discussed, and current controversies are highlighted. The reader will find precise answers to most relevant clinical questions. Rare topics, such as the occurrence of multiple primary tumors and the difficulty and occasional impossibility of distinguishing them from metastases, are also addressed. The role of surgery in patients with stage T4 primary tumors with infiltration of the superior vena cava is critically reviewed. An issue rarely discussed in clinical textbooks is cost effectiveness. Here, methods are briefly reviewed, and the cost effectiveness of some therapeutic interventions, in contrast to screening programs, is emphasized. Despite the proven cost effectiveness of palliative chemotherapy in comparison with the best supportive care, global budget restrictions may not allow this treatment to be offered to all patients. Given the growing importance of health care expenditures and limited resources, an even more extensive review of this subject would be welcome in a future edition. The first section of this textbook is devoted to the biology and molecular genetics of lung cancer. Potential targets for future therapeutic interventions, such as ras and erbB2, epidermal growth-factor receptors, telomerase, and angiogenesis, are appropriately reviewed. The difficulties, challenges, limitations, and opportunities of studying lung cancer in experimental models are highlighted. This information, together with the sections on clinical issues, makes Lung Cancer: Principles and Practice a truly comprehensive and valuable textbook on a common and often ultimately fatal disease. I reviewed this book over a three-month period. For every clinical question that came up at weekly multidisciplinary tumor-board meetings, I could easily find appropriate references, reviews, and management recommendations. The quality of this book lies in the fact that a vast amount of information is divided into 66 concise, practice-oriented chapters, most of which do not exceed 20 pages. This allows one to consult the book even in a busy clinic and to elaborate the answers to more difficult questions at the end of the day, within less than an hour. As with any multiauthored book, there is some unavoidable redundancy, but in the context of the multimodal treatment of lung cancer, it is even welcome. The limitations of standard therapy and the importance of continued basic and clinical research are pointed out. Анотація: For many years in the field of paediatric oncology there was a relative dearth of reference books compared with that available to our adult medical oncology colleagues. In the late 1980s the first edition of Principles and Practice of Pediatric Oncology was published and it quickly became the pre-eminent paediatric oncology resource text. A multidisciplinary team approach has become standard practice for managing paediatric cancer patients and all those involved in this field could turn to this text for detailed information on the majority of disease processes that occur in this patient group. Subsequent editions during the 1990s only served to enhance the reputation of this tome, though the third edition did suffer from several chapters only having minor ‘cosmetic’ changes to the text. The eagerly awaited fourth edition however has had major changes to the organisation of both the general sections and chapters. The editors have made the inclusion of up-to-date data on the molecular biology of paediatric malignancies their primary stated goal for this edition. While new advances and discoveries will quickly overtake the information provided, the editors and the contributing authors have succeeded in summarising the explosion in genomic knowledge of paediatric malignancies that has occurred over the past five to 10 years. In particular the basic issues section provides a solid foundation of understanding of core aspects of childhood cancer. The majority of contributing authors are North American in origin, but the individual disease chapters have in-depth discussions reflecting the global experience of previous treatment protocols. There are still idiosyncrasies that are simply reflections of differences in approach to particular problems. For example, in the discussion on the management of tumour lysis syndrome, the use of urate oxidase is only briefly mentioned. However, these instances are few, and at worst serve as instigators for debate and consideration. As in previous editions, the editors have retained the sections and chapters on supportive care, late effects, clinical trials, and psychosocial issues. This reflects the fundamental philosophy of the multidisciplinary team approach to paediatric cancer. It is no longer a question of simply surgery, chemotherapy and radiotherapy. The focus on other inter-related issues has contributed to improving both the quantity and quality of life of our patients. The emphasis on clinical trials throughout the text serves as a timely reminder as to the import of further collaborative work (including multinational studies) to the advancement of our knowledge and understanding. This text has therefore maintained its status as the most significant single resource in paediatric oncology.

Анотація: It is a challenging enterprise to assemble a comprehensive textbook on prostate cancer that goes beyond what other books afford. Although there have been at least a dozen other treatises on the same or similar subjects, Prostate Cancer: Principles and Practice has achieved the goals outlined in its preface: a summary of basic science and its relation to clinical medicine and a state-of-the-art summary of the management of prostate cancer. The student of the science of prostate cancer and the physician who treats men with the disease are challenged by an almost insurmountably complex disease process. This cancer affects one man in six and causes the death of 3 to 4 percent of all men, but it is apparent histologically in more than half of men during their lifetime. A number of key questions pervade clinical practice and provoke seemingly unending conversations between clinicians and basic scientists. What are the factors that precipitate the histologic event? Which tumors are biologically significant and require treatment? How does an individual patient balance the risks entailed by the disease with the risks involved in treatment, given our inability to predict the outcome of untreated disease? The reader of this book will understand the complexity of the disease, not just at the most basic, cellular level but at the level of the patient-physician interaction. The editors have selected outstanding authorities to contribute to this book. The crown jewel is its first section, entitled "Biology." These seven chapters are not only comprehensive but also insightful. Robert Myers's chapter on gross and applied anatomy, for example, traces the historical advances in prostatic embryology as it relates to anatomy and function. His integration of observations by American and European anatomists -- including the often-forgotten yet seminal work of Gil-Vernet in Barcelona, Spain -- provides a clear understanding of functional anatomy, an essential element in comprehending the effects of treatment on urinary and sexual function. Similarly, the depth of detail in the chapters on the molecular biology and genetics of prostate cancer is superb. The paired chapters 5 and 6 provide one of the most comprehensive discussions of the evolution of the hormone-resistant cell and potential therapeutic targets. In addition to the first section, the first two chapters of the epidemiology section (8 and 9), which are comprehensive and well written, should be required reading. Many of the clinical sections make repeated references to the same few clinical trials, some of which are discussed in detail in four separate chapters. Such repetition occurs because few clinical trials have been initiated, and even fewer have been completed. At least three chapters, for example, discuss the role of adjuvant radiotherapy after radical prostatectomy. The only randomized trial comparing adjuvant radiation with observation after radical prostatectomy began in 1987, and because the trial is concerned with survival, results will not be available until about 2004. Many clinical trials of patients with prostate cancer have compared treatments that are quite similar (e.g., radiotherapy with and without hormones), but challenging studies that compare dissimilar treatments are only now beginning to be undertaken (Figure). The trial of the American College of Surgeons Oncology Group and the National Cancer Institute comparing radical prostatectomy with brachytherapy has only recently begun; results may be a decade away. Thus, the clinical sections of the book suffer from the lack of data resulting from inherent challenges to clinical trials: the hesitancy of physicians and patients to offer or enter such trials, the large numbers of patients required for prostate-cancer trials, the long follow-up required, the lack of validated surrogate end points, and the very real risk that treatments may change dramatically long before study end points are reached. Nonetheless, there is much good counsel in this book. Since many physicians treat patients from the inception of the disease until death, managing the multitude of age-related and disease-related symptoms and complications, quality-of-life issues are exceptionally important

Анотація: This book has been written primarily for postgraduates involved in the surgical management of gynaecological malignancy: this includes both trainers and trainees. It is pictorial in nature and includes a full description of the majority of investigative and surgical procedures required in the repertoire of the gynaecological oncologist. The book is intended to fill a gap in gynaecological sections of both your own collection and your library The bulk of the text relates to surgery and gives clear written and pictorial instruction. These chapters have been written by well-known and respected surgeons and their fellows, all of whom are authorities on their subjects. The book is very much an international collaboration, with authors from the USA, the UK, France and Germany. Whilst there are a plethora of practical texts relating to general gynaecology, there are few specific to gynaecological oncology; of these, this is the only one to be produced in colour and to include an investigative section. The investigative chapters pull together investigations performed by ourselves and by colleagues from other disciplines, the sections on computed tomography, magnetic resonance imaging, cystoscopy and stenting being previously accessible only in radiological or urological texts. The chapter on genetic and other screening methods for ovarian cancer provides insight into a rapidly expanding area within the subspecialty. This text is not disease-based, and so no chapter relates specifically to endometrial cancer. Approaches to this malignancy differ, but all the possible radical elements of treatment (eg, nodal dissection, laparoscopically assisted vaginal hysterectomy, etc) are included in chapters here. The text is deliberately short, with only essential references and further reading. Much emphasis is placed on the illustrations, all the work of one team of artists to maintain clarity and continuity of presentation. The radiological and endoscopic sections include photographs with line drawings to aid clarity. This book is therefore very much designed as a practical guide to investigation and surgery—a ‘cook book’. We hope you, the reader, will obtain as much from reading it as we have derived from producing it.

Анотація: There is perhaps no better model of the importance of multidisciplinary interactions in the care of cancer patients than that found in the management of gynecologic malignancies. From the critical importance of primary surgical cytoreduction of advanced ovarian cancer preceding the administration of cytotoxic chemotherapy to the now-standard use of cisplatin as a radiosensitizer delivered with external-beam radiation therapy in the treatment of locally advanced cervix cancer, oncologists involved in the care of gynecologic cancer patients must understand the optimal utilization of multiple treatment modalities. For more than 5 decades, the gynecologic cancer program at M. D. Anderson Cancer Center has been an innovative leader in helping to establish the standards of care in this group of malignancies and in developing novel surgical, pharmacological, and radiotherapeutic approaches to improve both the survival and quality of life of women diagnosed with a gynecologic cancer. This well-written and comprehensive text describes the current management of female pelvic tumors, with chapters authored by nationally and internationally recognized senior leaders in their fields, as well as by more junior M. D. Anderson faculty who will soon be responsible for the new advances that will, without question, characterize the future of clinical research in gynecologic malignancies.
